Living safely and independently in your own home has always been at the heart of Omnicare’s mission. And with the new Support at Home program launching on 1 July 2025, older Australians will have improved access to assistive technology and home modifications – making everyday life easier, safer, and more secure.


What’s Changing under Support at Home?

Under the current Home Care Package system, many people need to save up their budget over time to afford important home upgrades or equipment.


That’s changing under Support at Home.


A new, dedicated Assistive Technology and Home Modifications (AT-HM) Scheme provides upfront funding, separate from your regular care budget, based on your assessed needs – so you can get help when you need it, not months later.

What Can the AT-HM Scheme Help With?

The scheme supports access to equipment and home changes that improve safety, mobility and independence, including:

  • Mobility aids – walking frames, wheelchairs

  • Safety enhancements – grab rails, ramps, non-slip flooring

  • Personal care supports – adjustable beds, shower chairs

  • Communication aids – visual alarms, amplified phones

  • Kitchen and daily living tools – to support meal prep, cleaning and more


All items must appear on the approved AT-HM List and be assessed as supporting your ability to live safely and independently at home.


How Does It Work?

If you’re eligible, your aged care assessor will include recommendations in your support plan and assign one of the following funding tiers:

*Higher amounts may be approved in some cases. Participants may need to co-contribute depending on the item and their financial circumstances.
