Our Home Care Services
STEP 1
Check you eligibility
My Aged Care is one of the first places you’ll need to visit if you want to access government funded aged care services in Australia. You can check if you’re eligible for an Aged Care assessment by using the online My Aged Care eligibility checker or by calling 1800 200 422. Eligibility is based on factors like your health, how you’re managing at home, and any support you currently receive. You may be eligible for aged care services if you have: • noticed a change in what you can do or remember • been diagnosed with a medical condition or reduced mobility • experienced a change in family care arrangements • experienced a recent fall or hospital admission, and are: • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people). To know for sure if you are eligible, you will need to have an assessment. If you’re not comfortable with calling My Aged Care, you can ask someone to do it on your behalf. You simply need to contact My Aged Care and register the person as your representative the first time you call.
STEP 2
Apply for an assessment
If you’re eligible, apply for an Aged Care assessment, either online at My Aged Care or by calling 1800 200 422. The application will ask questions about you, your current situation and the services you’re interested in. If you’re not sure which services are right for you, check My Aged Care’s ‘Types of Care’ guide. You can also refer to Omnicare’s services to see what we offer. Handy tip: Be prepared with your Medicare card, contact details and 15-20 minutes to complete the assessment. Our Omnicare team can support you with this process if required. You can complete the application yourself or on behalf of a family member or friend. If you’d like to set up an ongoing support person, you can also do that here.
STEP 3
Be assessed
Assessments are done in person at your own home. You don’t need to go anywhere. You will be contacted by an assessment organisation, and they will arrange a time that suits you. The purpose of the assessment is to better understand your needs. To ensure you’re prepared for your assessment, including knowing which questions to ask the assessor, visit the My Aged Care ‘How to Prepare for Assessment’ guide or call one of our friendly team on 1300 336 488. Handy tip: We recommend inviting a support person to be present at the assessment. This can be anyone you want, such as a family member, friend or carer. After completing your assessment, you will either learn the outcome immediately or be contacted by the assessment organisation within 2-6 weeks. You will then be placed on the National Priority System (NPS) to wait for your Home Care Package to be assigned to you. You will receive a referral code when your package is assigned to you.
How else can I use my funding?

Your Home Care Package is transferrable, or you can request your current provider “broker” some of Omnicare's services for you. For example, you may choose to have our meals delivered to you, or use some of your funding towards improvements in and around your home utilising our Modifications and Maintenance team. Omnicare can also arrange professional medical and allied health services, if you require them.
